The download is specific to your car only.

After 3 years if you want to continue getting maps you will need to pay an annual fee, again that will be on the bmw connected website.
Not looked into detail for mine yet but that be bundled with possibly things like RTTI , need to check on that.
Because the US is so large, the map updates are over the air but only region specific. You can go to the website and change your region whenever you want, however, if you want the entire map update you have to buy the USB stick with just under 30GB with the current 2019-2 map update.

The map update license is free for the first 3 years; however, you still have to buy the media if get it from BMW. You can also find places that will sell you a download version which is what I did.
