To your original question, Rule IX of the Rules of the House of Representatives govern expulsion of members of the House of Representatives. You might want to look into it. It's actually a higher standard than the bar needed to impeach the President.

Yes all 50 states get to decide they all vote and have the electoral college. If one state loses they dont usually spend the next 8 years making up things for an impeachment at the expense of finding a sane candidate.

Can't pretend to know if there is a chance of removing Pence after a theoretical Trump removal, but, it will be tried. He will be almost as reviled as Trump if he steps into President. There is social upheaval going on and Pence is seen as sort of an evangelical.

IF Trump were removed before 2020, Pence would simply be voted out in that election. No impeachment necessary.
