Rattling Noise - Brake Plate?

a week ago i had a severe rattling noise coming from the rear of the car. it sounded like dragging a bunch of tin cans underneath the rear axle. After having it picked up and taken to a dealer it turns out it was a bent rear drivers side brake plate.

7 days later - same noise happens again today.

1. Has anyone else had a similar issue or noise?
2. Was it a brake plate?
3. What would cause this to keep happening?
